What should I bring?
- Insurance Card (we'll need to take a copy, front & back)
- Subscriber Information: Name, Employer, & Date of Birth
- Applicable Co-pay
- List of Current Medications
- Physician Referral (if required by your insurance)
- For those under 18 year of age: a parent or legal guardian must be present (proof of guardianship required).
I'm a New Patient, what will you need from me?
In order to establish a patient account we will be asking you to give us the following information:
- First, middle, & last name
- Address
- Alternate phone number
- Gender
- Date of Birth
- Social Security number
- Employer
- Marital Status
If married, we will also need:- Spouse's name
- Date of Birth
- Employer
What should I expect?
Expect to arrive 10 minutes before your appointment time so that we may create your patient account, or update an existing one.
Expect that our Patient Coordinators will ask you if any information has changed since your last visit. Please inform them of a change of address, marital status, insurance, etc. With current and correct information the possibility of coverage denials can be minimized.
Expect that we may ask you to complete a health history questionnaire prior to your appointment.
If you haven't already received one, expect to receive a copy of our Privacy Policies and be asked to initial and date a receipt.
Expect to be asked to sign a Consent for Care form permitting us to treat you and bill your insurance; if your insurance fails to pay, you will be responsible for the charges.
Expect that a family will typically be set up as one account unless otherwise indicated by the patient.
